It is not often families understand clearly what’s needed to adequately support their loved one in the aging experience and Connect Our Elders is here to provide empathetic, proactive education and navigation through the many resources that exist. Simply providing a list of resources to these families is not adequate. It does nothing to reduce overwhelm. However, making a confident, professional, direct recommendation does.

What is a Care Manager?

The role of a care manager is multifaceted and complex.

A care manager serves as the main point of contact between the senior client, their care providers and family members.

There are a variety of roles a care manager may be responsible for, including:

  • Coordinating medical treatment, including administering assessments, developing care plans, monitoring medication compliance.
  • Relationship building with senior clients and family members.
  • Serving as advocates for and champions of the client’s health.
  • Connecting senior clients with social and community support systems.
